Man Charged For Forging Rx for Controlled Substance, Police Say

Also, a woman involved in a traffic crash charged with not having a driver's license. An arrest is merely an accusation, not evidence of a conviction.

Manish G. Patel, 20, of Glenview was charged with felony forgery Friday in a deceptive practice case, when he attempted to pick up a prescription at a pharmacy in the 9600 block of Milwaukee Avenue.

The pharmacist said he knew Patel as a regular customer, and became suspicious when Patel came in with a prescription for a controlled substance in someone else's name, according to the report. The pharmacist then called the a nurse practitioner at Advocate Lutheran General Hospital to verify the prescription, and she told him--and she later told police--that when Patel came in to the emergency room for treatment to an injury July 3, her prescription pad was missing from her work station after he left, the report indicated.

Police found Patel in his vehicle in the pharmacy parking lot, and the pharmacist made a positive identification of him as the person attempting to pick up the prescription. According to the report, Patel made statements indicating his involvement in the missing prescription pad. An assistant state's attorney approved a felony charge for forgery. His court date was July 9 at the Cook County courthouse in Skokie.

Woman charged with DUI

Vanesa Albizurez, 26, of North Meade Avenue, Chicago, was charged with DUI June 30 just before 4 a.m. when police stopped her for driving without headlights on and driving 55 mph in a 35 mph zone, according to the report. When police asked her if she had been drinking, she said she had. Police smelled the odor of alcohol on her breath, and she showed impairment on field sobriety tests, the report indicated. Her court date is July 26 at the Cook County courthouse in Skokie.

Woman involved in crash charged with no driver's license

Eileen Croft, 31, of Northwest Highway, Chicago, was charged with driving without a valid driver's license after she was involved in a traffic crash June 28 just after 8 p.m. in the 7100 block of Harlem. She was northbound on Harlem when she struck a car attempting to turn left, according to the report. A database check showed her license had expired in January 2012. She was taken by Chicago Fire Department paramedics to Resurrection Hospital, and Niles police told her to come to the Niles Police Department when she was released, which she did the following day. Her court date is July 23 in Skokie.
